What Is Community Palliative Care?

The Community Palliative Care Program (CPC Program) is an in-home service comprised of an interdisciplinary team of certified and trained palliative care professionals with a goal to improve the quality of life for people struggling with serious illness. In the most compassionate way possible, they provide disease management and serious illness education while addressing the physical, dietary, intellectual, emotional, social, and spiritual needs of the GCHP member and their family.

Livingston provides advanced care planning and assistance with medical care navigation by coordinating care with community providers and specialists caring for the member. Through personalized medical education the patient can engage in preventative strategies to reduce avoidable emergency room visits and hospitalizations.

At its foundation, palliative care aims to improve quality of life by taking a patient- centered approach to balance someone’s values and beliefs with their serious illness.

What Does Our Program Provide?

The Livingston Community Palliative Care Team includes physician assistants, nurses, dieticians, physical therapists, social workers, chaplains and community health workers. Every team member offers compassionate, patient-centered care and have specialized training and expertise helping people feel better through serious illness.

Who Can Refer A Patient To The Program?

Referrals can come from any provider who is caring for a GCHP Member. Our goal
is to create a supportive, respectful, and collaborative relationship with community primary care providers by extending compassionate in-home care to improve the patient’s quality of life and value of medical care. To support collaboration and open communication, all provider notes will be sent to the primary care provider.
