Livingston Earns 5-Star Hospice Rating from Medicare

A Mark of Excellence in Compassionate Care

Ventura County, CA — Livingston Memorial Visiting Nurse Association has received a prestigious 5-star rating from the Centers for Medicare & Medicaid Services (CMS) for its excellent hospice care, placing Livingston among the top-performing hospice agencies in California. Fewer than 1% of hospice providers in the state achieve this distinction.

Livingston is only one of 11 in the state of California that has achieved this highest rating out of 1,599 hospices statewide. It is also the only hospice in Ventura County to receive five stars.

The five-star rating—the highest possible score—is based on results from the Consumer Assessment of Healthcare Providers and Systems (CAHPS) Hospice Survey, a nationally administered survey completed by family members of patients who received hospice care. It evaluates key aspects of service such as emotional and spiritual support, communication, respect, and overall experience.

This five-star designation reflects the commitment of the entire hospice care team—nurses, social workers, chaplains, volunteers, and administrative staff—whose compassion and expertise define the Livingston experience.

Founded in 1947, Livingston Memorial is a non-profit home health and hospice agency that has already served more than 3,000 patients in Ventura County in 2025. Livingston Memorial began offering Medicare-approved hospice care in 1986. It is one of only six non-profit hospices in the state to be this highly rated.
