• Recopila con precisión los datos requeridos (OASIS) como parte de la evaluación al inicio de la atención (SOC), reanudación de la atención (ROC), traslado, alta, cuando hay un cambio significativo en la condición del paciente y cada sesenta días.
• Determina la elegibilidad del programa, los servicios necesarios y verifica las fuentes de los pagadores.
• Revisa los medicamentos del paciente en cada visita, mantiene una lista de medicamentos actuales y concilia la lista de medicamentos de Home Health con las listas de medicamentos de otros proveedores; por ejemplo, alta hospitalaria.
• Desarrolla y mantiene un plan de atención individualizado centrado en el paciente utilizando habilidades de pensamiento crítico. Evalúa la respuesta del paciente a la atención, actualiza el plan de atención y realiza derivaciones a recursos internos y externos para satisfacer las necesidades del paciente.
• Desarrolla e implementa un plan de enseñanza individualizado para el paciente y su familia/cuidador con el objetivo de ayudar al paciente a alcanzar el mayor nivel de independencia posible.
• Obtiene oportunamente órdenes médicas para la atención y los servicios necesarios.

• Manages Wound Advisor, verifies that each wound type and protocols are appropriate and current. Initiates revisions to plan of treatment as necessary.
• Using Wound Advisor, a software program that allows photographs of patients’ wounds to be uploaded to the e-record, is able to provide consultation to clinicians and physicians regarding wound care.
• Manages Wound Advisor protocols and makes changes as appropriate.
• Mentors staff in use of Wound Advisor.
• Provides educational classes for the staff as needed in coordination with the Director of Education.

• Participates and instructs in specialty related inservices for the organization and external professional organizations/associations as appropriate/required.

This position is responsible for continuity of physical therapy treatment and instruction of home health care patients, under the direction of a Registered Physical Therapist, to detect, assess, prevent, correct, alleviate and limit physical disability, movement, dysfunction, bodily malfunction and pain from injury, disease and any other bodily conditions in accordance with accreditation requirements, Title 22, licensure regulations and the organization’s policies and procedures.

• Performs physical therapy treatment as prescribed by the referring physician under the supervision of a Registered Physical Therapist, and in accordance with the LMVNA procedure manual and reviews treatment plan as indicated.

This position is responsible for coordinating referrals between assigned facilities and Livingston for the purpose of home care and hospice, educates facility staff, physicians, patients, and families about home health and hospice services, and participates in community outreach activities.

• Valid California driver’s license and automobile insurance per agency’s policy.
• Two years recent home health, hospice or acute care experience
• Experience in marketing, liaison activities, public and customer relations, and home health, home care and hospice functions preferred.

• Screens referrals for appropriateness to home care and hospice.

• Provides the agency with social and medical information about the referred patient to ensure continuity of care, assignment of appropriate staff and accurate information for medical record review and coding.
• Coordinates with facility discharge planners to ensure hospital/facility information is provided to LMVNA’S Intake Department in a timely manner.
• Communicates appropriately with the Intake Department to ensure continuity of care.
• If available, reviews facility census daily to identify existing LMVNA patients and assure the continuum of care to those patients on service with LMVNA.
• Participates in patient case conferences at LMVNA and assigned facilities.
• Ability to navigate and work within hospital/home health electronic computer system.
• Works in tandem with Livingston leadership to coordinate and participate in community activities such as presentations, health fairs and expos.

Position Summary:

Responsible for the implementation and daily operations of the Volunteer Program, including but not limited to; recruitment of volunteers able to fulfill a variety of assignments, ensuring the volunteers receive and complete required training to perform their assigned duties; supervision of the volunteers incorporating guidance, support, direction and accountability; ongoing documentation of volunteer activities; ensuring volunteer recordkeeping is up to date and compliance is maintained; processing and making assignments in response to requests for volunteer assistance; overseeing the Hospice Volunteer Coordinator; collaborating with staff at all levels in the agency; engaging in the internal and external coordination of events, projects, and department requests.

Qualifications and Requirements:

• Bachelors’ Degree or equivalent experience in the volunteer/nonprofit field. 
• Previous volunteer management/coordination. 
• Previous hospice or medical organization experience preferred. 
• Bi-lingual (Spanish) preferred.

• Tracks processes such as criminal background checks, TB screening forms, orientation paperwork, and number of volunteer hours work for cost savings reports. 
• Manages the Hospice Volunteer Coordinator 
• Collaborates with other departments to organize and facilitate special events/programs such as Grief and Bereavement programs, community education, health fairs, We Honor Vets, marketing, publicity, and fundraising events. 
• Coordinates the continuing education and engagement activities for volunteers and encourages volunteer participation. 
• Responsible for ensuring that volunteers submit their documentation, including hours, patient care records, and HR materials.
