This position works with the Certified Wound Ostomy Continence Nurse Specialist II providing consultation to clinical staff regarding wound management and ostomy care regarding patients on care with home health and hospice. This person acts as a consultant in a hospital setting in accordance with state licensure requirements, accreditation guidelines, Title 22 regulations, and the organization’s policies and procedures.

  • Assumes responsibility for the supervision and accountability of all aspects of hospice patient care services provided by CHHA staff.  
  • Will assume the responsibilities of the Department Director in their absence.  
  • Admits patients to all levels of hospice: GIP, Facilities and Residential. Provides as needed visits to hospice patients and completes all documentation necessary.   
  • Hospice office triage of patient needs, assisting clinicians in the field and performing office duties; arrange for DME and medication delivery. Manage and register pharmacy details.

  • Interacts with the Department Director, Patient Care Coordinators, Volunteer and Bereavement Coordinator, Chaplains, and Music counselors to oversee delivery of patient services and compliance with agency policy and all applicable regulations.  
  • Reviews medical records assuring accuracy, completeness and compliance with licensing regulations, certification standards, legal and ethical standards.  
  • Coordinates resources in a cost-effective manner while ensuring quality outcomes and requirements for payment.   
  • Cooperates and collaborates with Compliance/Quality Improvement Department to obtain quality-based outcomes in all areas of clinical performance. Assist with the development and implementation of quality improvement mechanisms that assure safe quality services to patients and their families.  
  • Works closely with the Director of Hospice to manage and promote hospice within the community to further LMVNA goals.

On Call Registered Nurse - Hospice (Weekends)

RESUMEN DE POSICIÓN:

This position provides timely nursing care visits to address the immediate needs of Livingston’s home health, palliative care and hospice patients at their place of residence after hours. Visits are determined by Livingston’s office staff or the after hours on call triage nurse. This position functions under limited supervision and requires excellent problem-solving skills in critical situations. All services are provided under a licensed physician’s order and in accordance with the Medicare Conditions of Participation, California Standards, and LMVNA policies and procedures must be followed.

  • Provides nursing services starting Friday at 4:30 p.m. until Monday at 8:00 a.m. Makes both scheduled and unscheduled visits.

ESSENTIAL JOB FUNCTIONS

  • Performs occupational therapy assessment and treatment as prescribed by the referring physician and in accordance with the Livingston procedure manual and revises treatment plan as indicated.

  • Demonstrates understanding of Livingston and third party payor or treatment guidelines and documentation required for payment. 
  • Instructs Livingston personnel as needed. 
  • Provides communication to Occupational Therapy Assistant (COTA) regarding patient’s care plan, supervises care provided by COTA, performs COTA field evaluation

Speech Language Pathologist

This position is responsible for the assessment, continuity of treatment and instruction of home health care patients who have speech language, communication, swallowing disorders, y/or cognitive dysfunction in accordance with accreditation requirements, Title 22 licensure regulations and the organization’s policies and procedures.
