In this session, we will review the Physician Orders for Life-Sustaining Treatment (POLST) and DNR forms—important medical documents that allow individuals to express their preferences regarding life-sustaining treatment in the event of a medical emergency. You’re welcome to take a copy of the form home, and we encourage you to review and complete it with your healthcare provider at your next visit.
