HOME TELEHEALTH CARE
TeleCare
TeleCare is an innovative program that offers remote medical oversight by a registered nurse. It is a private pay service available to clients who no longer qualify for Livingston Memorial Visiting Nurse Association skilled care and/or clients who desire medical supervision over an extended period of time.

Ideal Candidates for the TeleCare Program are clients who have:
- One or more hospitalizations in the past 90 days.
- More than two chronic conditions.
- Poor understanding of their disease process.
- More than 3 prescribed medications and/or medications that are not refilled in a timely manner.
- Decline in cognition.
- Decline in mobility & functionality.
- Lack of appetite, poor eating habits and/or impaired bowel function.
- A history of anxiety or depression.
TeleCare medical oversigh program is initiated by a registered nurse.
The initial assessment includes:
- A physical Assessment.
- Medication Reconciliation.
- Home Safety Evaluation.
- Recommended for appropriate level of Medical Oversight. (Can include comprehensive utilization of a computer and/or video monitoring system placed in the home)
Subsequent to the initial assessment and as indicated on future assessments, family members, as well as the client's physician(s), are given medical updates on the client's health condition.
