TeleManagement

TeleManagement is an innovative program that provides supplemental care to patients currently receiving services from Livingston Memorial Visiting Nurse Association.

TeleManagement is designed to monitor a patient's health status via scheduled telephone calls in conjunction with skilled clinician visits.

Registered nurses contact patients via telephone to assess overall health status, reinforce disease management education provided by licensed clinicians and intervene on newly identified health issues. During the telephone contact, nurses reconcile medication, asses blood pressure, weight, medication compliance, pain, nutritional status and bowel function.

Physicians are given medical updates regarding the patient's condition on a regular basis and when there is significant change in the patient's condition.
