HOME TELEHEALTH CARE
How It Works
Upon approval from the patients attending physician to participate in the Home Telehealth Program, and after it has been determined that the patient meets eligibility requirements, we contact the patient by phone and explain the Home Telehealth Program.
- Patient consent is obtained
- A video or non-video Telehealth monitor, blood pressure monitor, pulse oximeter, weighing scale and blood glucose meter, if needed, are installed in the patient’s home.
- The Telehealth RN instructs the patient how to measure his/her blood pressure, pulse rate, oxygen saturation, weight, and blood glucose, as the case may be.
- The patient demonstrates competence in obtaining the physiological measurements and in sending these measurements electronically.
- Patient and nurse expectations are explained to the patient and /or caregiver to ensure a satisfying Telehealth experience.
- Patient video encounter or telehealth visits by phone are provided twice a week for four weeks and once a week thereafter.
- The physician determines the guidelines for vital sign parameters.
- The Telehealth RN assesses the patient’s heart and lung sounds when interactive audio video is used through the use of a special stethoscope
- The physician receives patient’s vital sign trends weekly and as needed if vital signs fall outside set parameters.
- The patient may participate in the program for a maximum of 180 days, if the patient remains in a home health episode of care.
