Only patients admitted to our Home Health service with the diagnosis of congestive heart failure or other cardiac diseases and chronic obstructive pulmonary disease
Patients with a history of multiple hospitalizations and emergency department visits.
Patients taking ten or more prescribed medications; we help them adhere to the medications
Patients need to have a working phone landline to transmit the data successfully
Patients need to have manual dexterity to operate the Telehealth equipment
Patients must be alert and oriented.
Patients must be able to hear and see
An available and willing caregiver is required if the patient is unable to demonstrate functional and cognitive abilities to participate in the program
