What Is Home Telehealth?

The Home Telehealth Program provides remote care delivery to chronically –ill patients in their place of residence. This program combines a technology solution and extensive patient education delivered by phone or video to supplement the in-home visits made by LMVNA field clinicians.

We specialize in providing care to patients with congestive heart failure and other cardiac diseases, including chronic obstructive pulmonary disease and diabetes. We provide the best possible technology and skilled nursing services to detect early signs of complications using interactive audio video and non-video monitoring. By using up-to-date telecommunications technology and by providing telephonic or video Telehealth visits delivered in the comfort of the patient’s home we increase our patient’s access to skilled, empathetic and solution-oriented nursing services.

We obtain physician’s approval for all patients admitted to our program and patient’s consent to participate. We ensure privacy of patient information throughout the patient’s participation in the program.
