HOME HEALTH CARE
Our Care Team
Livingston Memorial Visiting Nurse Association caregivers are specially selected and trained to help in time of need. LMVNA caregivers include both medical and volunteer members. Below is a list of the types of caregivers and services we offer.
Physician Services Hospice services are provided under the supervision of the patient's personal physician and the Livingston Memorial Visiting Nurse Association Medical Director.
Nursing Services Registered nurses and licensed vocational nurses make regularly scheduled home visits to make assessments, teach patients and family members (or other caregivers) the needed care, and assist in controlling pain and other symptoms.
Home Health Aides Certified home health aides provide assistance with personal care needs.
Spiritual Support The LMVNA chaplain is available to make home visits or assist in contacting a clergy of choice. The chaplain will help the patient/family by drawing upon their own spirituality.
Volunteers Specially trained LMVNA volunteers provide emotional support and respite time for the patient and family.
A Complete Care Team As needed, the entire LMVNA dedicated care team is available to service hospice patients.
Therapists Physical, occupational, and speech therapists are available to improve home safety.
Social Workers Licensed social workers work with the care team to provide the patient/family with emotional and spiritual support.
